Birth Certificate

Just starting national process. Does a birth certificate (certified or otherwise) need to accompany the affidavit of citizenship evidence, or is that only for the passport process or other processes?

Hi antcamp21, you don't need to send your birth certificate with your affidavit, just the cover letter. You are correct; you will need to send it with your passport application.

I need to clarify my statement on submitting your birth certificate with your passport application. You will need to submit your birth certificate with the DS-11 passport application. This is the NEW passport application.
