How to start the process?

What are the steps to get started with my affidavit?

  1. Write your affidavit, make several copies and have them all notarized.
  2. Write a short cover letter and mail with one copy of your affidavit to:

    U.S. Department of State
    2201 C St., NW Washington, DC 20520

    ATTENTION: ADMINISTRATIVE RECORDS DEPARTMENT

    Mail certified with green card signature receipt.

  3. Mail out notices to your sheriff, mayor, coroner, chief of police, district attorney, county supervisor, public health, city council, election supervisor, child protective services and any others you feel need to be put on notice.
  4. Update your passport.

If I already have my affidavit in with the SOS should I also submit a copy of my affidavit when I get my passport & passport card

 

Yes, send a copy of your affidavit and your affidavit cover letter with your passport application.

We are getting our passports in person at a post office. When we have our affidavits & cover letters with us is there a concise verbiage that we should say while getting our passports & cards

The DS-11 and the DS-82 both have the same declaration that you’ll need to agree to when you sign the application. Both will refer you to the “warning” box on page four of the instructions. In the warning box it indicates that you can attach an affidavit to the application (this will be your Affidavit of Citizenship Evidence) even though it is not specific to this in the warning.

So, the only thing that you could do, is point this out to the person taking your application that you are attaching your documents with your application. The warning box gives you the right to attach an affidavit. 

To confirm that your affidavit was received with your application, we have another process that you can do. Look on the Passports page under Confirmation.
