I understand, no problem.

But the national status is about removing yourself from their jurisdiction. Why would you want to add information to your affidavit that puts you back into their jurisdiction? You can structure your affidavit anyway you want as long it is truthful. We intentionally leave the county of and state of, language off so we are not confusing the whole point of the affidavit.

Also, you can structure your notary section any way you want. We had no problems getting ours notarized. 

Just a side note: I also, am not a lawyer and we do give legal advise. But I was a notary a few years ago, and I was never taught that it had to have any particular verbiage. As long as they presented valid ID I could notarize for them.

Bottom line, draft your affidavit in any format you want. We are just presenting what we know works and follows Roger's method.
